If you are uninsured, underinsured, are on Medicare or Medicaid, or have questions about the cost of care, we are here to help. SMC offers financial assistance for patients through payment plans and a Sliding Fee Scale Program. We also have a MNsure Navigator on staff who can answer questions about insurance programs and assist in the application and enrollment process.

If you are uninsured or have questions about your insurance coverage, we can help. Call and ask to speak to our MNsure Navigator who is available to meet one-on-one with individuals and families.

Insurance contracts are subject to change. Please call the number on your insurance card if you have questions and to verify that our clinic is in your network.
COVID-19 initial evaluation and testing are covered services with no charge to patients. Testing and initial evaluation services for COVID-19 are covered under patients health insurance, and or Sliding Fee Scale. No charges will be billed to the patient for these services. For more information on COVID-19 charges please call the clinic.
For insurance companies: Commercial insurance will be billed at the following rate: E/M $113-$505 and COVID-19 test $0. Lab fees are additional through North Shore Health Hospital.
